Nature of Activities
Petite Playroom, operated by 1000457668 ONTARIO CORPORATION, is an indoor play facility that provides children and families with opportunities to participate in imaginative and recreational play activities. These activities may include, but are not limited to, soft play, role-play stations, physical movement, interactive play equipment, drop-in play sessions, private birthday parties, group visits, and other age-appropriate recreational programs for children.
Assumption of Risk, Release of Liability & Indemnification Agreement
In consideration of being permitted to enter and use the premises, facilities, equipment, and services of Petite Playroom, I, the undersigned, on behalf of myself and any accompanying adults and/or the minor child(ren) for whom I am the parent or legal guardian, acknowledge, understand, and agree to the following:
I understand that participation in activities at Petite Playroom involves inherent risks, both known and unknown, which may result in injury, illness, emotional distress, property damage, paralysis, or death to myself, accompanying adults, and/or the minor child(ren). These risks may include, but are not limited to, slips and falls, muscle strains or sprains, bruises, cuts, abrasions, broken bones, joint or ligament injuries, concussions, eye injuries, or other serious injuries.
I acknowledge that participation is voluntary, and I knowingly and freely assume all risks, whether foreseeable or unforeseeable, associated with participation in activities at Petite Playroom.
Release and Waiver of Claims
On behalf of myself, accompanying adults, and/or the minor child(ren), and to the fullest extent permitted by law, I hereby waive, release, and discharge Petite Playroom, 1000457668 ONTARIO CORPORATION, and all related parties, including owners, directors, officers, employees, contractors, volunteers, agents, affiliates, successors, and assigns (collectively, the “Released Parties”) from any and all claims, demands, actions, damages, costs, or expenses (including legal fees) arising out of or related to participation in activities at Petite Playroom, including those resulting from the negligence of the Released Parties.
I agree that I will not bring or initiate any legal proceeding against any of the Released Parties for any such claims. I further agree to indemnify and hold harmless the Released Parties from any claims brought by or on behalf of myself, accompanying adults, or the minor child(ren).
Medical Treatment
I understand that if medical treatment is required as a result of participation, such treatment will be obtained at my own expense or through my personal insurance. I agree that Petite Playroom and the Released Parties are not responsible for any medical costs or related expenses.
Acknowledgement of Health & Conduct
I confirm that I am aware of any pre-existing medical conditions or injuries affecting myself or the minor child(ren). I understand that participation may aggravate such conditions and I voluntarily assume this risk, waiving any related claims.
I acknowledge that Petite Playroom reserves the right to immediately end participation if I, accompanying adults, or the minor child(ren) are unable to follow instructions, comply with safety rules, or behave in an unsafe or inappropriate manner.
Rules & Safety
I confirm that I have read, heard, or been informed of all rules and safety guidelines applicable to participation at Petite Playroom.
I agree to comply with all posted and communicated rules and understand that these rules are in place for the safety of all participants.
Photo & Media Release
I grant Petite Playroom permission to capture and use photographs, video, or audio recordings of myself and/or the minor child(ren) taken while on the premises. These materials may be used for promotional, marketing, advertising, training, or other lawful business purposes, without compensation. I understand that all such content remains the property of Petite Playroom and waive any right to review or claim ownership.
Acknowledgement
By signing this waiver, I confirm that I have read and fully understand its contents, and that I am signing it freely and voluntarily on behalf of myself and any accompanying adults and/or minor child(ren).
